Church Forms

Here are links to some of the forms the church uses. For PDF files, you will need to download the free Adobe Reader if you don't already have it installed on your computer. A link is located to your left.

Check Request Forms (PDF File)
Use this form if you need to be reimbursed or pay a vendor directly for a church-related expenditure. Be sure to attach legible receipts.

Connection Partner Information Form (PDF File)
This form is used in outreach to recent prospects/visitors.

Facility Use Request Form (Church Events) (Microsoft Word Template)
Use this form if you need to reserve a room or area of the church for a church-related function. It is provided to you in a Word template and you can open and type in the shaded areas to complete it. You can save the file to keep a copy for your records, and then email it as an attachment to melindaw@brookwood.org.

Facility Use Form for Non-Church Events (Microsoft Word Template)
Use this form that a church member can use to request use of the facilities for non-church related events. Examples: Scout Troop Meetings, Neighborhood Association Meetings, etc. It is also provided in a Word template so you can type in the shaded areas to complete it. You can also save the file for your records and email it as an attachment to melindaw@brookwood.org.

Family Medical Information & Release Form (PDF File)
This form is used for families with children or youth who participate in events (day trips, camps, mission trips, etc.) sponsored by Brookwood Baptist Church. One form may be completed annually for the entire family. It is the responsibility of the parents to alert BBC of any changes to the information.

Food Service Request Form (Microsoft Word Template)
Use this form to let the church know of food and related items needed for church-related events, i.e., paper goods, coffee, etc. This form is provided for you in a Word template, so you can type in the shaded areas, save it for your records, and email it as an attachment to melindaw@brookwood.org.

Meal & A Deal Donation Form (PDF File)
This form is used by individuals or businesses who donate items for the silent and live auctions of this annual fundraising event that benefits the summer missions work of the Youth Ministry. Completed forms should be returned ASAP to the Youth Ministry Office or faxed to 967-0481.

Mission Funding Form (Microsoft Word Form)
Each group or ministry that desires funding for mission projects must complete and submit this form.

Mission Quarterly Report Form (Microsoft Word Form)
Funded mission projects should use this form to report quarterly on their activities, revenues, and expenditures.

Youth Council Application Form (PDF File)
Youth may use this form to apply to become a member of the Youth Council.
